Located in New Haven County, Connecticut, PACT-Hartford HealthCare’s current members comprise several divisions covering multiple specialties: internal medicine, gastroenterology, general surgery and nephrology. While we remain committed to primary care as the core of our group, we are seeking to grow and diversify to ensure the long-term strength of our practice.

A growing suite of health services in Connecticut

Since 2020, PACT-Hartford HealthCare has partnered with Hartford HealthCare, which has close to 400 locations and affiliated healthcare facilities throughout Connecticut. This partnership gives PACT-Hartford HealthCare access to a much wider network. Hartford HealthCare’s nearly 400 locations serve 185 towns and cities. Its specific resources include two tertiary-level teaching hospitals, an acute-care community teaching hospital, an acute-care hospital and trauma center, three community hospitals, the state’s most extensive behavioral health network, a large multispecialty physician group, a clinical care organization, a regional home care system, an array of senior care services, and a large physical therapy and rehabilitation network. Hartford HealthCare utilizes a unique, system-wide Institute model that offers a single, high standard of care in crucial specialties at hospital and ambulatory sites across Connecticut. The institutes include: Cancer, Heart and Vascular, Ayer Neuroscience, Orthopedics and Tallwood Urology.

In addition, all PACT-Hartford HealthCare members are all on the Epic Electronic Health Record system, with implementation, training and support provided by PACT MSO. PACT-Hartford HealthCare plans to add additional ancillary services to continue to add value and revenue opportunities for PACT-Hartford HealthCare members.
