The PACT MyChartPLUS is a secure online website that gives PACT patients convenient, 24-hour access to personal healthcare information via the Internet. Using a secure username and password, patients can view healthcare information such as:
- Recent provider visit details
- Lab results
- Medications
- Immunizations
- Allergies
PACT patients can also use MyChartPLUS to:
- Request prescription refills
- Schedule non-urgent appointments
- Exchange secure e-mails with PACT healthcare providers
- Update contact information
- View educational materials through HealthWise
If you have not yet set up a NextGen Patient Portal account, enrollment is no longer possible; please contact the billing department to make a payment by phone by calling 203-488-7228 option 1 for dates of service March 31, 2021 and earlier.
