Rheumatology is the medical specialty focused on conditions that affect the joints, muscles, bones, and immune system. A rheumatologist diagnoses and treats arthritis, autoimmune disorders, inflammatory diseases, and other conditions that may cause pain, stiffness, swelling, weakness, or changes in mobility.
A primary care provider can evaluate common aches and injuries. A rheumatologist may be needed when symptoms persist, inflammation is suspected, bloodwork suggests an autoimmune condition, or a chronic disease requires specialty treatment and monitoring.
PACT Rheumatology treats a broad range of inflammatory, autoimmune, joint, and bone health conditions.
An autoimmune form of arthritis that can cause persistent joint swelling, stiffness, pain, and reduced movement.
A systemic autoimmune condition that may affect the joints, skin, kidneys, blood, lungs, and other parts of the body.
A form of inflammatory arthritis that commonly causes sudden, intense joint pain and swelling due to uric acid crystal buildup.
Conditions involving reduced bone density that may increase fracture risk and require testing, treatment, or continued monitoring.
A chronic condition associated with widespread pain, fatigue, sleep disruption, and increased sensitivity throughout the body.
Mixed connective tissue disease (MCTD) has features of multiple autoimmune disorders and may affect the joints, muscles, skin, lungs, or other organs.
An inflammatory condition that primarily affects the spine and may cause back pain, stiffness, and changes in mobility.
Evaluation and treatment for immune system disorders that may cause inflammation in the joints, connective tissue, skin, or internal organs.
Treatment for inflammatory and degenerative forms of arthritis, with attention to pain, mobility, disease activity, and long-term joint health.
Biologic medications and infusion therapy may be used for certain autoimmune and inflammatory conditions when clinically appropriate.

The doctor took his time to review my medical history. After explaining what he thought it may be he went over what we had to do next. Never felt rushed and always had the answers to my questions.
The staff is professional, caring and well trained. Dr. Puchalski is an excellent physician who is a strong diagnostician and looks at the totality of his patient’s medical issues, not just the pulmonary aspect of their condition. I am beyond pleased by the care and treatment I received.
Was a pleasure meeting the Doctors! Explained everything in an excellent way and answered all my questions!
My pulmonologist was excellent; he took his time to review all my records; explained my condition and possible future solution(s) to my issues in the future. Dr. Puchalski is a wonderful doctor – knowledgeable, experienced, kind and very meticulous and dedicated.
Dr. Puchalski was very helpful and was open to my helping me with my health. He did the tests and we talked about that. My lungs were clear.
“Dr. Puchalski was my doctor at Yale after I was diagnosed with cancer on the lining of my lung. I had a buildup of fluid, called a pleural effusion, that had to be extracted. Dr. Puchalski did a great job extracting the fluid from my lung and also performed a tube thoracostomy that I needed. He inserted a catheter into my pleural space to continue to drain the fluid. Dr. Puchalski was always informative, treated me in a professional way, and, a year later, made me laugh as he pulled out the tubing I was so petrified of having him do. I highly recommend him.”
Joint pain, swelling, stiffness, or unexplained inflammation may require evaluation by a rheumatologist. PACT welcomes new patients at its Hamden, Guilford, and Orange offices.
Rheumatology is the branch of medicine focused on diseases that affect the joints, muscles, bones, connective tissue, and immune system. Rheumatologists treat conditions such as arthritis, lupus, gout, osteoporosis, and autoimmune disease.
A rheumatologist diagnoses and treats conditions such as rheumatoid arthritis, psoriatic arthritis, lupus, gout, fibromyalgia, Sjögren’s syndrome, ankylosing spondylitis, osteoporosis, mixed connective tissue disease, and other inflammatory and autoimmune conditions.
You may need to see a rheumatologist if you have persistent joint pain, swelling, stiffness, unexplained inflammation, muscle weakness, recurrent gout, abnormal autoimmune bloodwork, or symptoms affecting multiple parts of the body. A referral may also be appropriate when a primary care provider suspects an inflammatory or autoimmune condition.
Referral requirements vary by insurance plan. Some patients can request an appointment directly, while others may need a referral from a primary care provider or another physician. Contact your insurance company or PACT Rheumatology before scheduling.
Your first visit may include a review of your symptoms, medical history, medications, previous lab results, and imaging. The rheumatologist may examine affected joints or muscles and recommend bloodwork, imaging, or other testing. Treatment recommendations depend on the findings and suspected condition.
